New Employee Forms

There are a number of start-up and benefit-related forms a new employee must complete, including employees who are new hires, re-hires and internal hires to the Government of Saskatchewan.

The Online Commencement Process in Taleo is to be used for every hire, unless the candidate is unable to access the internet. It identifies and is used to initiate each step in the offer process including the online completion of candidate start-up documents (benefits enrolment, direct deposit, etc.).

If the candidate does not have access to the internet their start-up forms will need to be completed manually. Completed forms are to be sent to the Human Resource Service Centre to be processed. Forms for the manual process are listed below.

In-Scope SGEU Employee

There are two possible start-up packages for in-Scope SGEU employees – re-hires/new hires or internal hires.

Re-Hires and New Hires

The re-hire/new hires SGEU start-up package includes employees that are permanent full-time, permanent-part time, term, labour service, student and students that are less than full-time.

• Benefit Plan Enrolment Form (Extended Health Care and Dental)
• SGEU Membership Registration Form

Internal Hires

If you are an internal hire moving from CUPE or Out-of-Scope to SGEU, please complete:

• Group Life Insurance Enrolment Form (SGEU)
• SGEU Membership Registration Form

In-Scope CUPE Employee

The CUPE start-up package should be completed by re-hired or newly hired in-Scope CUPE employees. This includes permanent full-time, permanent-part time, term, labour service, student and student less than full- time employees.

In-scope CUPE start-up package includes:
